The Ottawa County Health Department can issue certified Birth Certificates for individuals born anywhere in Ohio from December 20, 1908, to the present date.

The health department maintains death certificates for those who died in Ottawa County starting December 20, 1908, to present date.

Application for Certified Birth or Death Certificate

When requesting a birth or death certificate by mail, please send the application form and a check or money order made payable to the Ottawa County Health Department, along with a self-addressed stamped envelope.
