Helping people connect with care

As a community-focused and community-supported organization, it’s important to all of us at Community Hospice & Palliative Care to give back to the communities we serve in equal measure to what they provide us. We do this by investing significant resources in community-focused programs, as well as outreach and education. The goal is to ensure all families get the care and support they need, regardless of ability to pay.

It’s a fact that most people don’t seek out a palliative or hospice care provider until they need one, either for themselves or a loved one. Often, needed care and support—and their benefits— arrive much later than they could.

The Community Hospice & Palliative Care Neviaser Educational Institute provides education and other resources for health care professionals and the community about dealing with serious illnesses and their impact. The Community Hospice & Palliative Care Foundation generates philanthropic and community support for patient care services, family needs and community programs, supplemented with contributions raised by the Community Hospice & Palliative Care Thrift Shops (www.thriftshopjax.com). In addition to providing hospice care, Community Hospice & Palliative Care offers several programs that help improve quality of life, such as Community PedsCare, a nationally recognized pediatric palliative and hospice program for children with life-threatening and life-limiting conditions; Community Hospice & Palliative Care Veterans Partnership, a program providing specialized care and support to address the unique needs of Veterans; Community Palliative Care, a consultative service for people with serious and complex illnesses that helps them make important health care decisions; and Community Care Choices, a CMS research model to help people with advanced illness find greater access to comfort care and support.
