Case Management services are client-centered with the goal of assisting with management of symptoms and keeping individuals functioning as independently as possible.  Individual choices and needs are emphasized while providing community-based services and promoting independence and the pursuit of meaningful living, working, learning, and leisure-time activities. Healthy lifestyles are encouraged through wellness coaching and promotion of community integration. 

Clients may participate in case management through our Community Psychiatric Rehabilitation Program (see Community Psychiatric Rehabilitation Program for service description). On rare occasions when consumers do not meet the diagnostic or financial eligibility for traditional Community Psychiatric Rehabilitation Program services, but could benefit from case management services they may enter into a private agreement for a case manager.

This service is available as requested on a private-pay basis for persons with serious mental illness or serious emotional disturbance. The case manager will assist the client by linking them with ongoing needed services, skill building, illness management or relapse prevention. A request for service would be followed by a traditional intake screening and then assignment of a case manager.
