Housing and Health Opportunities – H2O
The Housing and Health Opportunities (H2O) Demonstration, available through Arizona Complete Health Complete Care Plan beginning October 1, 2024, offers eligible members access to housing support services and community resources. This initiative is designed to enhance overall well-being by addressing housing as a key component of health. In collaboration with provider housing specialists and Solari, Arizona Complete Health Complete Care Plan helps eligible members coordinate their care with network providers, ensuring a more integrated and supportive approach to both health and housing needs.
