Caris Transitions, an innovative program by Caris Healthcare, has been designed to optimize communication and ensure seamless access to hospice services for patients when the time is right.

We understand that reaching out to Caris is a call for assistance, and if a physician refers a patient who may not be eligible or prepared for hospice services, we extend our support through Caris Transitions. As a compassionate resource, we remain committed to assisting the patient and their family throughout their journey.
